Hospice of Southern Maine is proud to host a series of Community Forums to guide community members on understanding and completing the Maine Health Care Advance Directive Form. End-of-life care decisions are some of the most difficult and complex decisions many of us will ever make. These free community forums will be facilitated by expert hospice and bereavement staff, bringing highlights from this year’s Thresholds Conference into the community. Based on Thresholds keynote from award-winning journalist, bestselling author, and former Good Morning America host, Joan Lunden, and the panel discussion with southern Maine medical experts, these forums will provide a step-by-step guide on care decisions needed to complete the Maine Health Care Advance Directive form.
